For any individual, receiving a memory impairment diagnosis can be overwhelming and challenging, raising concerns and fears about their future.

Individuals worry about how the disease will progress, how it will affect their independence, and how it will impact their relationships with family and friends. These concerns are compounded by the increased challenges in managing daily activities, along with navigating a healthcare system that requires you to be your own advocate.

At The Memory Compass, we realize that each person's experience is unique, and there is no one-size-fits-all solution to this problem. Our Cognitive Wellness Services help individuals navigate this new reality by providing them with information, tools, and support that is personalized to their specific diagnosis and personal environment.

Our service provides you with a roadmap that you can follow going forward. We are proud to provide the "missing link" of guidance and support for day to day challenges following a diagnosis of Mild Cognitive Impairment (MCI), dementia, Alzheimer's disease, Vascular Dementia, Lewy Body Dementia, Frontotemporal dementia and Parkinson's Disease.

Needs Assessment and

Initial Consultation

Before your initial consultation, you will complete a background and needs assessment that will guide our holistic approach to your Cognitive Wellness Plan. Family and care partners can also complete a needs assessment to strengthen your support network.

 

During our initial consultation, we will walk through your needs assessment with a focus on ensuring that we are providing tools, systems, and supports tailored to your concerns.

Personalized Cognitive Wellness Action Plan

After walking through your needs assessment at the initial consultation, your memory loss navigator will build your personalized Cognitive Wellness Plan, targeted to your specific needs and concerns.

The Cognitive Wellness Plan will serve as the foundation of your care plan and a main reference point as we build out strategies and actions to empower you on your journey.

Routine Check-Ins and Ongoing Support

After delivering your personalized Cognitive Wellness Plan, we will meet with you on your preferred frequency, whether that be weekly, bi-weekly, monthly, or quarterly. These sessions can also include extended family and/or care team members.

These sessions will be spent reviewing your care plan, discussing recent events or concerns, and evaluating new challenges as your needs evolve. We are your advocate and here to help you navigate along the way!
