Cognitive Screenings

Our Cognitive Screenings are provided by a dementia care specialist, certified dementia practitioner, and master's level healthcare professional with a background in speech-language pathology with advanced training in assessing and providing treatment for executive function, memory, and other cognitive impairments. 

Have you been experiencing cognitive changes and wondering whether it was normal aging or a sign of something more? Or, do you have a family history of dementia or Alzheimer's disease and interested in a screening to gain a baseline and a better understanding of ways to optimize your brain health? 

Our comprehensive cognitive screenings are ideal for: 

  • Individuals who are concerned about whether cognitive changes they may be experiencing are a part of normal aging or a sign of something more
  • ​Adult children or family members who have a history of dementia or Alzheimer's in their family and are interesting in having a baseline of their current cognitive function 
  • Individuals who are interested in learning more about brain health and how lifestyle factors contribute to brain health 

Our Cognitive Screening

  • New clients will complete our new client intake document (click below) to gather information on personal history, lifestyle factors, and any current concerns

  • Your consultant will review this document with you during your 1:1 consultation and obtain additional information (stress, mood, diet, etc) in order to gain a more holistic view of your needs

  • The screening assessment will take 30 - 45 minutes to complete

  • Once the cognitive screening assessment is complete, you'll receive an individualized report that is yours to keep

An Individualized Brain Health Plan

  • Following the results of the comprehensive screening, our clients have the option to have a follow-up session to create a cognitive wellness plan individualized to your unique lifestyle and needs

  • This will include further in-depth education on brain function

  • Further 1:1 guidance on how to optimize your brain health 

Virtual and Phone Services Available

Access our services from the comfort of your home.

Clients across the state of Indiana and the United States can access our cognitive screening services and our cognitive wellness services virtually from the comfort of their own homes! Virtual services are an excellent way to receive quality education, guidance and support for individuals who do not have access to expert care in their area. During our 15-minute discovery call we will ensure a person is an appropriate candidate for online services before beginning. ​We also offer these services by phone.

We will also assist in onboarding for virtual services.
